Everyday Wellness with Chelsea Weight Loss Membership Consent & Disclaimer Form

1. Acknowledgment of Services

I understand that Chelsea Priddy is a Weight Loss and Wellness Coach and is not a doctor, registered dietitian, or licensed nutritionist. The coaching provided is for educational and informational purposes only and is intended to support and guide me through the Everyday Wellness with Chelsea Weight Loss Program.

2. No Medical Advice

I understand that:

  • The coaching and materials provided are not intended to diagnose, treat, cure, or prevent any disease.

  • The information provided is not a substitute for professional medical advice, diagnosis, or treatment.

  • I should consult with my physician or qualified health provider before beginning any weight loss program, exercise program, or lifestyle change, especially if I have any medical conditions.

2a. Pregnancy and Breastfeeding Policy

I understand that I am not eligible to participate in the Everyday Wellness with Chelsea program if I am currently pregnant or breastfeeding.

2b. Eating Disorders Disclaimer

I understand that Everyday Wellness with Chelsea is a coaching program and does not replace professional medical or mental health care. If I have a history of, or am currently struggling with, an eating disorder, disordered eating behaviors, or any mental health condition related to food or body image, I understand that I should seek professional help before participating.

3. Personal Responsibility

I understand and agree:

  • I am fully responsible for my own health and well-being.

  • I will listen to my body and will not push myself to discomfort or harm.

  • I will consult my healthcare provider regarding any questions or concerns I have about my health before making any changes based on this program.

  • I am ultimately responsible for reaching out to Chelsea Priddy to book my 1:1 appointments, attend sessions, and actively participate in the group community.

4. Assumption of Risk

I acknowledge that participation in any weight loss, nutrition, or wellness program involves inherent risks including physical or emotional discomfort, and I voluntarily assume all risks associated with participation.

5. Release of Liability

In consideration of participation, I release and hold harmless Chelsea Priddy and affiliates from any claims, demands, or causes of action that may arise from my participation.

6. No Guarantees

I understand that while this program is designed to support weight loss and wellness, no results are guaranteed. Outcomes depend on individual commitment, consistency, and other factors outside the control of Chelsea Priddy.

7. Confidentiality and Group Privacy

I agree to respect the privacy of other members and not share or distribute any personal information, stories, or details outside the group.

8. Intellectual Property & Program Materials

All materials provided are the intellectual property of Chelsea Priddy. I agree not to copy, share, sell, or distribute any program materials outside of this program.


9. Refund Policy

Because our program is virtual with immediate access to our weight loss program, refunds are only offered if you:

  • (1) Provide a doctor’s note, or

  • (2) Fully follow the program and are still unsatisfied.

To qualify for a refund without a doctor’s note, you must:

  • Attend your 1:1 Initial Session within 30 days

  • Attend 3 of 4 weekly 1:1 sessions with Chelsea

  • Attend 3 of 4 weekly Group Meetups

  • Submit 5 of 7 daily check-ins with your weight to Chelsea each week

  • Follow your customized plan (unless adjusted in a 1:1 session)

  • Engage weekly in the community

  • Submit before/after photos or measurements

If you meet all of the above criteria and are still unhappy, contact Chelsea for a full refund. This policy supports accountability while removing financial risk.

Supported Membership

Supported Membership

Our Supported Membership offers comprehensive, personalized support to help you achieve and maintain your weight loss goals. It includes:

  • Weight Loss Plan PDF
    A complete plan covering weight loss and maintenance, relapse prevention tools to help you stay on track, and general wellness resources.

  • 1:1 Initial Coach Meeting (60 minutes)
    A personalized session with Chelsea to review your goals and create your custom weight loss plan.

  • Weekly 1:1 Coach Meetings (15 minutes)
    Ongoing weekly coaching calls with Chelsea to track progress, make adjustments, and stay accountable.

  • Daily Messaging Support (Monday–Friday)
    Send daily weight updates and receive support or guidance as needed for accountability and progress. (Less frequent check-ins available upon request. Communication via community messaging or email.)

  • Weekly Group Meetings
    Participate in a live group call each week focused on applying tools and resources, asking questions, and reflecting on your week by discussing your recent wins, any setbacks or barriers you encountered, and your motivation or focus for the upcoming week.

  • Access to the Online Community
    Connect with peers for support, encouragement, and recipe ideas. Includes access to a live chat for in-the-moment connection.
